Disclaimer & Assumption of Risk

OPTIMUM PROFESSIONAL ADVISOR PRIVATE LIMITED

1. Acknowledgment of Inherent Risk

Participation in sports, athletic training, physical conditioning programs, and competitive events inherently involves certain risks of physical injury, including, but not limited to, muscle strains, broken bones, serious long-term disability, or in rare instances, death.

By enrolling in or participating in any program offered by the Mission Olympics Sports Academy, the participant (or the parent/guardian, if the participant is a minor) acknowledges and voluntarily assumes all risks associated with participation in these activities.

2. Health and Fitness Representation

The participant affirms that they are physically fit and have no medical condition that would prevent or limit their safe participation in the activities provided by the Academy. Participants are strongly advised to consult with a licensed physician prior to beginning any new physical training regimen.

3. Waiver of Liability

To the fullest extent permitted by law, the participant, on behalf of themselves and their heirs, executors, administrators, and assigns, hereby releases, waives, discharges, and covenants not to sue the Mission Olympics Sports Academy, its directors, officers, coaches, employees, volunteers, and agents (collectively, the "Released Parties") from any and all liability, claims, demands, actions, or causes of action whatsoever arising out of or related to any loss, damage, or injury, including death, that may be sustained by the participant, or to any property belonging to the participant, while participating in such activity, or while in the Academy’s facilities or grounds.

4. Limitation of Liability for Property

The Academy is not responsible or liable for any personal property lost, stolen, or damaged on the premises or during Academy-sponsored events. Participants are advised not to bring valuable personal items to training sessions.

5. Photographic Release

Unless a specific written opt-out form has been submitted per the Privacy Policy, participants consent to the use of their image, likeness, voice, or performance in any photographs, video recordings, or audio recordings taken by the Academy for promotional, archival, or training review purposes without compensation.

6. Governing Law

This Disclaimer and Assumption of Risk shall be governed by and construed in accordance with the laws of [Insert Jurisdiction/State/Country].

By signing the registration form, the undersigned acknowledges that they have read this Disclaimer and Assumption of Risk, fully understand its terms, and understand that they are giving up substantial rights, including their right to sue.
